The Maryland Insurance Administration approved rate increases for four insurers offering on-exchange health plans next year. Baltimore-based CareFirst BlueCross BlueShield — which holds 68 percent of the state’s health insurance market — will increase rates an average of 31 percent for preferred provider organization plans and 24 percent for its health maintenance organization plans.

The smallest increase the administration approved is Baltimore-based Evergreen Health Cooperative’s roughly 20 percent increase, up from its nearly 9 percent requested increase. The co-op owes $24.2 million in risk adjustment payments to the federal government.

About three-fourths of Maryland residents receive federal subsides to offset premium costs, according to the report.
