CarePort Health to manage Allscripts' care management portfolio: 4 things to know

Boston-based CarePort Health assumed management of Allscripts' Care Management solution and services portfolio April 10.

Here are four things to know about the management change.

1. CarePort Health, which Allscripts acquired in 2016, provides post-acute outcomes management software to ACOs and hospitals nationwide.

2. Allscripts' Care Management solution and services portfolio comprises acute case management, discharge planning, utilization review and referral management platforms.

3. CarePort Health, including the Allscripts' Care Management portfolio, plans to grow to 200 full-time employees by the end of 2018, officials said.

4. The merged portfolio will offer providers, payers and ACOs visibility into care management across the care continuum.

"The shift to outcomes-based reimbursement models created a heightened need for providers and health organizations to transition patients out of the hospital efficiently with established solutions, such as Care Management, and to track patients' health as they transition across multiple care settings, using technologies such as CarePort," explained Lissy Hu, MD, CEO and founder of CarePort Health.
