Two Minnesota Healthcare Providers to Collaborate to Improve Care

HealthPartners, a Bloomington, Minn.-based HMO, and Minneapolis-based Allina Hospitals & Clinics announced that they have entered into a long-term collaboration to improve outcomes, satisfaction and affordability of care to patients in the northwest metro area of the Twin Cities, according to a joint news release by the two organizations.

The collaboration will involve financing from HealthPartners and four HealthPartners’ clinics in the northwest metro area as well as four Allina clinics and Allina’s Mercy Hospital in Coon Rapids, Minn. The collaboration includes:

•    a payment agreement that rewards quality, patient experience and total cost of care rather than paying just for individual units of service;
•    sharing electronic health information to coordinate care, improve efficiency and safety and improve the patient experience; and
•    activities between hospitals and clinics to coordinate care for patients with chronic conditions and reduce unnecessary readmissions.
