St. Luke’s Clinic Coordinated Care (Boise, Idaho). St. Luke’s Clinic Coordinated Care joined the Medicare Shared Savings Program in 2013 as a Track 1 ACO and transitioned to the Next Generation ACO Model in 2017. The ACO serves patients in Idaho and Oregon.
