Kaleida Health, ECMC and MASH CareNetwork announce new population health management model

A new population health management model designed to align hospitals and home care facilities with urgent care, transportation, physician services and mobility management is underway in Western New York.

The initiative to provide a seamless experience for patients is being developed by Buffalo, N.Y.-based Kaleida Health, the Erie County Medical Center in Buffalo and MASH CareNetwork, a health services provider.

Together, the three entities are working on coordinating transportation and dedicated resources so that hospitals can improve their respective discharge planning processes, getting patients home in a timelier manner. Additionally, the initiative will likely decrease hospital lengths of stay.

“We are very proud of this venture,” said Jody L. Lomeo, president and CEO of Kaleida Health. “The affiliation with MASH will mean better organization and management of our healthcare delivery system in a manner that makes it more clinically effective, more cost effective and safer for our patients. In short, it’s all about improving the patient experience.”

Transportation is just one of three initiatives that Kaleida Health, ECMC and MASH CareNetwork will undertake in 2015. Plans for a care coordination and mobility management center and a preferred diagnostic network for the local HMOs are also in the works.
