Heritage California ACO (Northridge, Calif.). Heritage California ACO is an affiliate of Northridge-based Heritage Provider Network. In 2012, CMS selected Heritage California to participate in the Pioneer ACO Model; in 2016 Heritage California transitioned to the Next Generation ACO Model. The ACO’s operator, Heritage Provider Network, contracts with 2,300 primary care physicians, 30,000 specialist physicians and more than 100 hospitals.
