Hill Physicians Medical Group (San Ramon, Calif.). Hill Physicians Medical Group participates in four ACOs across California: CalPERS ACO, City & County of San Francisco ACO, University of California Employees in San Francisco ACO and San Joaquin County ACO. Hill Physicians partnered with Dignity Health and Blue Shield of California on a virtual ACO model for 41,000 employees who receive CalPERS in 2010. The group also collaborates with Dignity Health, Blue Shield of California and the University of California San Francisco on an ACO-type arrangement for employees. Hill Physicians’ newest ACO, the San Joaquin County ACO, formed in March 2014 through a partnership with Blue Shield of California and Dignity Health’s St. Joseph Hospital — it covers around 15,000 members. In 2017, CMS selected Hill Physicians to participate in the Next Generation ACO Model.
