DaVita Medical ACO California. In 2012, DaVita acquired HealthCare Partners in a $4.42 billion deal and the combined organization launched DaVita Medical ACO. CMS selected HealthCare Partners, now a division of DaVita, to participate in the Next Generation ACO Model in 2017. The organization, which provides integrated care management for approximately 750,000 patients, implemented the Next Generation ACO Model in the California market.
