Heritage Provider Network, Rite Aid Announce Collaborative Care Agreement

Northridge, Calif.-based Heritage Provider Network and Rite Aid have entered into an agreement to bring the Rite Aid Health Alliance program to HPN’s chronic and poly-chronic patients across Southern California.

Under the agreement, Rite Aid and HPN will offer patients with conditions such as diabetes, heart failure, hypertension and high cholesterol coordinated care and support. Rite Aid pharmacists and HPN physicians will help the patients meet wellness goals and improve their overall health. 

“Known as a pioneer in population health management, Heritage Provider Network has long believed in and offered a wide array of innovative and preventative healthcare services, making them a natural partner for us as we continue to bring this one-of-a-kind healthcare program to the communities we serve,” said Ken Martindale, president and COO at Rite Aid, in a news release.

Thirty Rite Aid pharmacies, eight HPN affiliates and HPN’s accountable care organization will be participating in the program.
