Walgreens, Heritage Provider Network Partner For Population Health Management

Deerfield, Ill.-based Walgreens and Heritage Provider Network, headquartered in Northridge, Calif., have entered into a clinical care collaboration agreement.

Under the agreement,  Medicare Advantage and ACO members who receive care at Arizona Priority Care — HPN’s Arizona operating affiliate — will receive coordinated care and expanded services through certain Walgreens pharmacies and healthcare clinics, according to a news release.

Arizona Priority Care providers and Walgreens pharmacists and healthcare clinic nurses will coordinate patient care, with the goal of improving patient care while reducing costs.

“Coordinated care programs are vitally important to help ensure patients have access to the quality care they need, especially in today’s healthcare environment,” said Jeffrey Kang, MD, senior vice president of health and wellness services and solutions at Walgreens, in the release.
