Health insurer Cigna has announced three new collaborative accountable care initiatives, its version of accountable care organizations: two in Connecticut and one in New Hampshire.

The payer partnered with Core Physicians, a multispecialty group with more than 120 physicians in Exeter, N.H., PriMed, a multispecialty group with more than 120 physicians based in Shelton, Conn., and The Greenwich (Conn.) Physicians Association, an independent practice association with 106 physician members.

All three CAC initiatives will have care coordinators, who will use patient-specific data from Cigna to help identify patients who may be at risk for readmission, skipped a prescription refill or are overdue for screenings. They can also help schedule appointments and provide health education, according to the news releases.

As part of the arrangements, Cigna will pay the physician groups for medical care and care coordination services. Also, the groups can be rewarded on a pay-for-value structure if providers meet quality and cost targets.
