The Sutter medical foundations include the Palo Alto (Calif.) Medical Foundation, the Sutter Medical Foundation, the Sutter Gould Medical Foundation, the Sutter Pacific Medical Foundation and the Sutter East Bay Medical Foundation.
Each entity has its own separate ACO arrangement with Anthem. The ACOs give enrollees “access to a personalized health team including a doctor, care coordinator and other practitioners need,” Anthem told the San Francisco Business Times.
The ACOs are available to fully ensured Anthem PPO patients with multiple chronic conditions.
