Aetna and WESTMED were able to reduce hospital admissions among their patients by 35 percent in the first year after forming their patient-centered medical home. WESTMED physicians also met or exceeded 90 percent of their targeted goals for diabetes management and screening, cancer screenings and heart disease, according to a news release.
“We are very pleased with the results of our medical home program with WESTMED, which is having an obvious positive impact on our members,” said Dave Kobus, senior vice president of Aetna’s New York Network, in the release.
Aetna and WESTMED are expanding their relationship and entering into a collaborative care agreement to support more than 650 Aetna Medicare Advantage members. Under the agreement, Aetna nurse case managers will work as a team with WESTMED physicians to coordinate care for patients with the goal of improving their health outcomes.
