VCA includes the following seven Connecticut hospitals and their affiliated physicians: Derby-based Griffin, New London-based Lawrence + Memorial Hospital, Middletown-based Middlesex Hospital, Bridgeport-based St. Vincent’s Medical Center and the Western Connecticut Health Network, which includes Danbury (Conn.) Hospital, New Milford (Conn.) Hospital and Norwalk (Conn.) Hospital.
Under the agreement, nurse case managers from Aetna will assist the ACO with care coordination, outreach and patient follow-up. Care coordinators from VCA will work with patients to help improve health outcomes. If VCA providers are able to meet quality and efficiency benchmarks that have been shown help improve health outcomes, they have the opportunity to share in savings.
