The partnership will use naviHealth’s decision support technology, post-acute care analytics platform and clinician network to improve patient outcomes and transition of care.
Through the partnership, the two companies will share data, collaborate clinically on ER diversion and readmission programs and including appropriate Golden Living providers in any provider networks established by naviHealth.
