Norfolk, Va.-based Sentara Health reported a data breach involving a July 7 mailing error at its behavioral health clinic in Harrisonburg, Va.
The breach occurred when Sentara behavioral health specialists mailed departure notices to patients of a specific provider. An entry error caused letters to be sent to incorrect addresses. Each letter included the recipient’s name, their provider’s name and practice location.
No other personal or clinical information was disclosed. Sentara said the breach was limited to patients of that provider.
Notification letters were sent Sept. 2. The health system disclosed the incident in a Sept. 5 news release and said internal action was taken in accordance with policy. It is also evaluating additional training to help prevent future errors.
