Clinicient’s new platform, Insight Care Navigator, will connect care coordinators to providers and equip administrators with the tools and data necessary to maximize each patient’s experience and level of care at a lower cost.
The platform includes tools to enhance patient communication and reporting dashboards that allow care coordinators to view patient outcomes and cost of care in real-time.
“Payers and providers are concerned about how they will coordinate care, measure clinical outcomes and control costs in value-based care arrangements,” Kent Rowe, Clinicient’s CEO, said in the press release. “Insight Care Navigator provides the simple, integrated tools to enable entire care delivery teams to work together to assure better outcomes at a lower cost.”
