Millennium ACO (Fort Myers, Fla.). Millennium Physician Group and ProCare Med formed an ACO in 2013 to treat Medicare beneficiaries across southwest Florida. It generated the sixth largest sum in shared savings, $7.98 million, in the Medicare Shared Savings Program in 2014. Millennium Physician Group has since joined Florida Blue’s ACO, which launched in 2015.
