Accountable Care Collaborative (Colorado). Colorado is home to the longest-standing Medicaid ACO, according to The Commonwealth Fund. Known as the Accountable Care Collaborative, this Medicaid ACO is organized into seven Regional Care Collaborative Organizations that develop networks of providers. These providers create medical homes throughout the state for Medicaid members. The ACC achieved more than $37 million in net savings for fiscal year 2014-15, according to the Colorado Department of Health Care Policy & Financing.
