Cerner announces first UK-based HealtheIntent population health management partnership

Wirral Partners, a U.K.-based integrated primary and acute care system, is the first to partner with Cerner to utilize its HealtheIntent population health management platform. Wirral Partners is using HealtheIntent to advance Healthy Wirral, the system’s population health initiative.

“Wirral Partners are passionate about improving the health and well-being of local people and are developing a new model of integrated care that provides holistic assessment and intervention to meet the health and social care needs of local people,” Jon Develing, executive lead for the Healthy Wirral program, said in a statement. “We believe this model will be further enhanced by informatics-enabled population health management.”

In March 2015, England’s National Health Services selected Healthy Wirral as one of 29 pioneer sites for improving care delivery models in the U.K.
