Advocate Cerner Collaborative (Downers Grove, Ill.). Downers Grove, Ill.-based Advocate Health Care and Cerner joined forces in 2012 to launch the Advocate Cerner Collaborative, a program focused on bringing population health insights into clinical workflows. Its team comprises clinical process designers, data scientists and researchers who develop and deploy data analytics solutions. Since its inception, the center has developed statistical models that predict the risk of hospital readmission; recommend appropriate discharge locations; and identify patterns related to medication nonadherence.
