Triad HealthCare Network (Greensboro, N.C.). Triad HealthCare Network holds various risk-based contracts with payers including Medicare, UnitedHealthcare and Humana, covering patients across five North Carolina counties. Seventeen Greensboro- based Cone Health employed physicians and neighboring independent providers coordinate care in the ACO, with Cone Health receiving at least $2 million of any shared savings accrued each year. Triad HealthCare Network joined the Medicare Shared Savings Program in 2012 and transitioned to the Next Generation ACO Model in 2016.
