TPN Health Partners (Omaha, Neb.). TPN Health Partners is a Medicare Shared Savings Program ACO that includes five hospitals operated by Englewood, Colo.-based Catholic Health Initiatives and The Physician Network. TPN is a wholly owned subsidiary of CHI and includes more than 50 primary, specialty and urgent care practices across Nebraska. The ACO was approved to participate in Track 1 of the Medicare Shared Savings Program in 2016.
