Sacramento, Calif.-based UC Davis Health tracks dozens of hypertension patients each month through remote patient monitoring.
Here are five things to know about the program:
1. The health system launched the initiative in 2024, checking the patients’ blood pressure from home via connected cuffs and scales, according to a March 26 news release.
2. The devices transmit data to the UC Davis Population Health team — made up of a physician, nurse, pharamacist and medical assistant — which uses a color-coded report to quickly flag abnormal readings.
3. “The RPM program helps us identify and reach patients quickly to adjust medications or address concerns before they become emergencies,” Brianne Harms, RN, lead triage nurse for remote patient monitoring at UC Davis Health, said in the release.
4. UC Davis Health signs up about 40 new patients each month, caring for approximately 180 at a time.
5. The program, which includes group classes, one-on-one coaching and medication help, lasts six months, but people can stay in it for up to a year.
