Specialists On Call unveils post-acute care platform for value-based care

Specialists On Call, a Reston, Va.-based telemedicine provider, launched a platform for post-acute care.

With the post-acute care platform, Specialists On Call supports telemedicine coverage across the care continuum, for acute and non-acute healthcare providers. The platform aims to improve chronic condition management, emergency department visit prevention and overall patient engagement.

“We developed this scheduled consult offering to enable our clients to extend their telemedicine reach outside the four walls of their hospitals,” said Hammad Shah, CEO of Specialists On Call.

The post-acute care platform is targeted toward improving value-based care outcomes for ACOs, integrated delivery networks and health systems running population management programs.
