The insurer has teamed up with Seniorlink to begin a six-month pilot test with up to 250 Medicare Advantage members and family caregivers across 34 states.
Humana created a Virtual Care Team to spearhead the program. Management teams in the Humana At Home division will connect digitally with Humana Medicare Advantage members and their families through Seniorlink’s Vela app.
The app allows Humana team members and patients to communicate and collaborate more effectively. In turn, Vela is designed to improve patient satisfaction, alleviate caregiver burden and reduce overall costs.
Humana is rolling out the program with its members that have multiple chronic conditions, struggle with daily activities and are frequently hospitalized.
