Where are Leapfrog's 12 'F' hospitals?

The Leapfrog Group doled out 12 "F" grades to hospitals for their patient safety in its spring 2023 rating. 

Leapfrog analyzed nearly 3,000 U.S. hospitals and graded each one based on each center's "ability to protect patients from preventable errors, accidents, injuries and infections," the organization said May 3.

Less than 1 percent of hospitals were given an "F." The 12 are:

  • Columbia Memorial Hospital (Hudson, N.Y.)

  • Hazard (Ky.) ARH Regional Medical Center 

  • Knox Community Hospital (Mount Vernon, Ohio)

  • Los Angeles Community Hospital

  • Nacogdoches (Texas) Memorial Hospital

  • Ochsner American Legion Hospital (Jennings, La.)

  • Pacifica Hospital of the Valley (Sun Valley, Calif.)

  • Roseland Community Hospital (Chicago)

  • Southwest Medical Center (Liberal, Kan.)

  • Stringfellow Memorial Hospital (Anniston, Ala.)

  • United Memorial Medical Center (Houston)

  • United Memorial Medical Center (Sugarland, Texas)

Five hospitals — Knox Community Hospital, Ochsner American Legion Hospital, Pacifica Hospital of the Valley, and the two United Memorial Medical Centers — earned an "F" in fall 2022. The others were downgraded from a "D" in the last ranking.
