Telemedicine Company to Unveil New Solution to Reduce 30-day Readmission Rates

 
BlitzCare, a telemedicine company, is announcing a new transitional care management solution to help hospitals reduce their 30-day readmission rates.

The solution will connect patients to their healthcare professionals 24/7. It will help make physicians available to answer any questions that patients may have, monitor their conditions, adjust their medications and/or dosages and refer patients to outpatient services.

The system aims at helping hospitals reduce their readmission rates, and thereby the readmission penalties imposed by CMS. The pilot program is set to launch in three New York-based hospitals in 2014.
