Study: EMR’s Risk Assignment Helps Cut Heart Failure Readmissions 19%

An EMR-based software platform stratified patients admitted with heart failure according to their risk for a readmission within 30 days. The hospital implemented a set of evidence-based interventions to the highest-risk patients to prevent a readmission. From 2008 to 2010, the readmission rate decreased 19.1 percent.
