The Geisinger Monitoring Program is a post-hospital discharge telemonitoring solution that was introduced as an adjunct to existing case management to reduce hospital readmissions. In a large-scale, two-year study, Geisinger compared 30-day readmission rates for heart failure patients who did and did not use the home telemonitoring program.
At the end of two years, the telemonitoring program reduced all-cause 30-day readmission rates by 44 percent, compared to a control group that had case management but no interactive voice response protocol.
Based on the success of the monitoring program with heart failure patients, Geisinger has since rolled out the service to patients with hypertension and diabetes.
