RARE Campaign Reduces Hospital Readmissions by 16%

Riverwood Healthcare Center in Aitkin, Minn., announced a 16% reduction in readmissions after working with the Reducing Avoidable Readmissions Effectively campaign.

The RARE campaign is a Minnesota-based organization with a threefold mission to improve health, improve the care experience and lower care costs. It partners with healthcare institutions to identify and address contributing factors in hospital readmissions.
 

 

RARE helped RHC achieve reduced readmissions by implementing a patient education program for those most at risk for readmission. RHC identified the most commonly readmitted conditions and discharged patients with those conditions with educational materials.
