The “Toolkit for Working With Family Caregivers” is adapted from the change package developed for the Transitions in Care-Quality Improvement Collaborative, an initiative of Next Step in Care that ran from March 2010 to June 2012. The toolkit includes resources related to medication, discharge planning and communication.
