Patient Safety Tool: Patient-Centered Medical Home Checklists, Tools

The American Association for Family Physicians has launched the PCMH Planner, an interactive tool for physicians interested in transforming their practices into patient-centered medical homes.

The AAFP has released the tools to help smaller practices that may not be able to afford practice facilitators better understand the conversion to PCMH. The online tool is educational and available for a nominal one-time fee.

In addition, the organization offers a number of free resources on the transition to PCMH, including a checklist, a list of initiatives, information on PCMH health IT and resources on patient-centered care.
