The Multi-Center Medication Reconciliation Quality Improvement Study, funded by AHRQ, has produced a medication reconciliation toolkit to reduce medication errors during care transitions. The Toolkit includes an improvement guide for medication reconciliation, training videos for taking medication histories and providing discharge medication counseling and a return on investment calculator for investments in medication reconciliation quality.
