Jersey City, N.J.-based LibertyHealth, which operates Jersey City Medical Center, offers a free congestive heart failure discharge guide for patients in English and Spanish.

With help from the American Heart Association, JCMC created this education guide to prevent readmissions by helping congestive heart failure patients understand their condition and post-discharge instructions. The guide, “Living with Congestive Heart Failure: A Guide to a Healthy Heart,” includes a weight chart and medication chart, among other tools, to help patients monitor their health post-discharge.
