The guide aims at helping healthcare facilities adapt or expand existing Medicaid readmission reduction efforts; develop a Medicaid reduction strategy using a roadmap with customizable online tools; Comply with CMS’ Conditions of Participation requirements; and develop partnerships across other settings of the healthcare continuum.
