The Joint Commission has developed two new resources to help healthcare providers work on reducing avoidable readmissions and also improve the discharge process.
The first resource is the "Speak Up: Avoid a return trip to the hospital" campaign. The campaign's materials, including a video and infographic, use language that is easy for patients to understand and are designed for healthcare professionals to display and distribute to patients on-site and online.
The second resource is a "Quick Safety" newsletter for healthcare professionals. The newsletter covers suggested actions for healthcare workers to improve the discharge process and help patients avoid a readmission.
