The fines were $25,000 per violation in 2008 and became graduated in 2009: $50,000 for the first violation, $75,000 for the second and $100,000 for the third or subsequent violation at the same facility.
The state has levied 134 fines against 90 hospitals, collecting $2.3 million, and 22 of the fines have been appealed since the fines were first implemented in Jan. 2007.
