Herbert R. McMaster Sr., 84, the father of former national security adviser H.R. McMaster, suffered from a serious head injury after an unwitnessed fall on April 12. He died April 13 in the lobby at Cathedral Village in Philadelphia while sitting in a wheelchair, Pennsylvania Attorney General Josh Shapiro said.

Police detained Mr. McMaster Sr.’s nurse, Christann Gainey, May 10, after surveillance video revealed she failed to complete eight required neurological checks following the fall. The neurological checks are administered to indicate the severity of a patient’s injury and help determine proper treatment.

When the assistant director of nursing requested Ms. Gainey’s neurological assessment paperwork, it became apparent Ms. Gainey’s last check was reported 20 minutes after McMaster’s time of death. Ms. Gainey admitted to Cathedral Village staff she falsified the final check.

Ms. Gainey faces charges of neglect of a care-dependent person, involuntary manslaughter and tampering with records.
