The guide was developed by an NQF-convened, multi-stakeholder committee. It is structured around 10 key elements, ranging from a community’s readiness to begin a population health program to prioritizing health improvement activities. The selected groups will have the opportunity to interact with one another and with members of the committee.
Anyone wishing to be considered as a field testing group can apply until Oct. 7. The submission form and additional information is available online.
