A state investigation found problems like excessive use of restraints, improper handling of blood transfusions and unsanitary storage of some scopes, according to the report.

The patients who were in the 30-bed hospital were discharged and some were relocated to other facilities in the area. The hospital’s roughly 70 employees were also affected by the closure.

The hospital will reopen some time at the end of May or beginning of June, according to the report.
