The most challenging requirement for ambulatory care in 2011 is granting, renewing or revising clinical privileges to individuals who are permitted by law and the organization to practice independently, with a non-compliance rate of 46 percent. The most challenging requirement for hospitals is maintaining complete and accurate records for each patient, with a non-compliance rate of 66 percent.
