To be eligible for the certification, organizations must be critical access hospitals or hospitals currently accredited or pursuing initial accreditation by The Joint Commission. They must also have ambulatory care services that include the provision of primary care services offered by a primary care clinician.
The PCMH certification requirements relate to the following five operational characteristics:
• Patient-centered care
• Comprehensive care
• Coordinated care
• Superb access to care
• Systems-based approach to quality and safety
The certification will be valid for up to a maximum of three years.
