Hospitals can review their data on the QualityNet Secure Portal, submit questions about calculations and request corrections for the following scores:
- CMS Patient Safety Indicators 90 measure score
- Central line-associated bloodstream infection measure score
- Catheter-associated urinary tract infection measure score
- Surgical site infection measure score
- Methicillin-resistant Staphylococcus aureus bacteremia measure score
- Clostridium difficile infection measure score
- Winsorized z-score measure results
- Total HAC score
To request a score review, hospitals can email hacrp@lantanagroup.com with the subject line: “HACRP: Scoring Calculations Review and Corrections Inquiry.”
