The two-year initiative rewards Westmed for continued and improved patient care coordination and outcomes in certain categories, such as cancer screening measures and diabetic or cardiovascular monitoring and care. The two groups will also share data in order to support appropriate patient care.
Westmed has earned Level 3 recognition from the National Committee for Quality Assurance PCMH Program, the highest level available.
This is Aetna’s first PCMH agreement in Westchester County, N.Y.
