The report, “Better to Best: Value-Driving Elements of the Patient Centered Medical Home and Accountable Care Organizations,” outlines the goals of medical homes and ACOs and the need for reportable measurements, a change in payment systems and learning collaboratives. The authors also recommend specific actions to implement the changes needed to make patient-centered medical homes and ACOs successful.
