The expansion will include 22 practices and 75 primary care physicians who care for more than 171,000 patients.
Highmark’s PCMH pilot program has shown that the patient-centered medical home model reduced overall care costs by roughly 2 percent, according to the release. It also demonstrated positive results in the reduction of seven- and 30-day readmissions.
