Optimus, Healthcare Partners, Horizon Blue Cross of New Jersey Link for New ACO

Optimus Healthcare Partners — a New Jersey accountable care organization founded by Summit-based Vista Health System IPA and Central Jersey Physician Network — has partnered with Horizon Blue Cross Blue Shield of New Jersey for a new ACO agreement covering 11 counties.

The agreement includes 42 primary care practices and 104 primary care physicians. Horizon will provide incentive payments to Optimus physicians if they improve the health and patient satisfaction of more than 40,000 Blue Cross members. Practices participating in the ACO will operate as patient-centered medical homes.

The ACO is open to Horizon commercial and Medicare Advantage members who use practices that are part of the new arrangement.
