Montefiore Medical Center Signs ACO Agreement with Empire BlueCross BlueShield

Montefiore Medical Center in New York City has signed an agreement to form an ACO with Empire BlueCross BlueShield.

Under the agreement, advanced care management services will be offered to Empire’s fully insured members in Bronx and Westchester counties as well as to Montefiore’s Administrative Service Organization plan administered by Empire.

As part of the pay-for-performance model, if quality standards are met, Montefiore will be eligible to receive additional compensation.

In 2011, Montefiore was selected by the Centers for Medicare and Medicaid Innovation to participate in the Pioneer ACO Program.
