Horizon BCBS of New Jersey Expands PCMH Program

Horizon Blue Cross Blue Shield of New Jersey has expanded its patient-centered medical home program to cover an additional 45,000 patients.

About 700 physicians and other care providers are now involved in the PCMH program, which started in January 2011.

As part of the program, Horizon BCBSNJ gives upfront funding to participating practices to hire population care coordinator nurses. Participating practices can also receive outcome-based payments if they meet certain goals for care quality, increased patient satisfaction and lower cost.

The program now covers a total of 200,000 Horizon BCBSNJ members.
